现代故事中，女主角索菲娅（全智贤 饰）与尼娜（李冰冰 饰）是要好的朋友，经常在一起玩耍、学习。
一次偶然机会，苏菲从姑姑口中听到一个故事：“很久以前在乡下，年轻的同龄女孩会彼此发誓成为老同。
这是女人间最亲密的关系，甚至超越夫妻、姐妹。
彼此沟通是靠写在扇面上的一种秘密语言，这就是女书。

时光回退到1800年中期的湖南小镇，这里生活着两个清秀可人的小女孩，百合（李冰冰 饰）与雪花（全智贤 饰）。
俩人在幼年时结为老同，用女书互诉真情。
随年龄增长，两人相继嫁夫生子。
原本贫苦的百合因外貌出众和有一双完美的小脚，嫁入富家；而生于富家的雪花却因为父亲吸鸦片而家道中落，下嫁给乡间屠夫（姜武 饰）。
百合夫家父母双亡，她顺理成章成为当地最有权势的村长夫人；雪花却在艰苦的生活中挣扎，还要承受丧子之痛。
为了照顾雪花，百合恳请她搬离屠夫家与自己同住。
雪花爱夫心切，拒绝了百合的好意。
